| Refused to work and why? Was another job done in place of this? | He did not refuse to work. His supervisor Ronald has addressed concerns about him being confused as he wanders away from the assigned area he is to clean | ||||||
| Did the person arrive on time, if not why? | Yes he arrived on time. At the end of his shift his supervisors has to track him down just to get him to clock out on time but he has been found in areas he isn't assigned to and tends to clock out after 9 pm | ||||||

| What duties were performed today? | He was assigned to clean the stairwells but he has been found in other parts in the building without his equipment confused about where he is suppose to be and what he is suppose to be doing. They will guide him back to his area but they can not watch over him the entirety of his shift due to their own obligations | ||||||

| Did person have all necessary items to perform job duties? | He is given the proper equipment to do the assigned task for each day depending on if it's mopping or sweeping. The issue has become he wanders off from his equipment and has be misplacing them when he does so | ||||||
| Did the person interact positively with coworkers? | He interacts well with his co-workers and other patrons that he may come upon in the building. His supervisors have been interacting with them more lately due to his wandering and having to locate him to bring him back to the section he was assigned to, they have stated that he has been confused at times when they approach him. | ||||||
| While working, did person take initiative to move on to another task without prompting? | He hasn't been taking the initiative to move to other tasks because he has been wandering off from his tasks and it isn't known if he is actually completing his tasks | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | Incidents of him wandering off from his assigned areas and his confusion has been cause for concern. I have spoke to Natasha regarding the concerns the supervisors have and has scheduled a meeting with his coordinator to address these concerns and what we can do to assure his employment | ||||||
