| Comments, Office/DDS Follow Up | Driver stated when he dropped him off that Sammy needed to use the bathroom, which is normal for him when he first gets to work, but also added to watch him because he will take off all his clothes. I have never seen him with all his clothes off when he is using the restroom and is taking a while, and when I do go to check on him he has his clothes on he just tends to leave the stall door open while he is using the bathroom. He had a ham and mayo sandwich, cheddar goldfish, great value strawberry yogurt, a vanilla boost drink and a bottled of water for lunch | ||||||
