Assessment
Refused to work and why? Was another job done in place of this?No he didn't refuse to work, kept asking to go to lunch as soon as he got in to distract from the training
Did the person arrive on time, if not why?Yes he arrived an hour earlier
Was the person appropriately dressed, if not why?Yes he has on white Tshirt with a red sleeveless shirt over top, black sweatpants and his blue Nike shoes
What duties were performed today?Tried to keep him focus on his tutorial and training videos but he used lunch and asking me what I am doing to keep him distracted
Assessment Continued
Did person have all necessary items to perform job duties? No he still doesn't have his tablet that he was using before. He used that as a distraction as well saying he had t o call his cousin to sell the ones he do have to him so he can get a new one
Did the person interact positively with coworkers? He interacts pretty well, He has been making these loud cat noises and making claw gestures. Also as an observation when it seems I'm on the computer that is when he likes to start asking aggressive questions as what to what I am doing, or asking about do I have enough money to buy him something, or just asking questions that aren't relevant to these tasks at hand. I have discussed and said certain questions can make some people uncomfortable suggesting that once he does start working to be careful of the things he may ask of his co-workers, than directly telling him that his questions are inappropriate and sometimes uncomfortable to me as well
While working, did person take initiative to move on to another task without prompting? No he does not you have to constantly remind him to stay focus otherwise he will distract himself with lunch and other means
Were there any incidents/health related issues/medication side effects or concernsNo incidents to report at this time
Comments, Office/DDS Follow UpTrying to keep him focus has been a hard task. Keeping him to appropriate questioning in conversation has been a task because he does ask personal questions of me.
Risk Assessment MonitoringAt risk for falls due to his aspiatic gait and seizure disorder
