| While working, did person take initiative to move on to another task without prompting? | Mr White needed some prompting moving from one area to another once he passed that area before. He needed additional reassurance that he would find debris in areas he previously swept. |

| Comments, Office/DDS Follow Up | Mr White maintains very good focus during his shift. He still needs to concentrate on being aware of people behind him. At times he was a bit stubborn about moving out of the way of some customers. This need an additional encouragement. He does recall some of these incidents and has made some adjustments. |
