| Refused to work and why? Was another job done in place of this? | Mr.McAdams did not refuse any work that was assigned to him. If anything Mr. McAdam would refuse to keep his breathing machine on the entire work day. He would find a way to remove it while working and say he ok for that moment. However staff always remind Mr. McAdam that his breathing machine is for his best interest. Then he eventually puts it back on. |

| What duties were performed today? | Mr.McAdams duties included maintenance the mens restrooms. He supposed to wipe down toilet and urinals. Restock toiletries, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock wipe the floor if wet as he is leaving out each restroom. |

He stocked his cart full of color coded rags to wipe different surfaces, liquid soap to replenish the soap dispenser. Trash bags for the trash, paper towels and toilet paper as well as a dry mop. |

| Comments, Office/DDS Follow Up | Mr. McAdams had a productive day at work covering all the men's restrooms for both west and east wings of the building. Mr.McAdams did remove his breathing machine from his nose twice this evening while working. The first reason he gave staff that the bag was uneven and it was interfering with his work because it was uncomfortable. The second reason he just said he ok and would be able to work with if off for that specific floor however staff fixed the issue with the bag being uncomfortable by adjusting the shoulder straps. As well as convince Mr. McAdams that he continues to remove his breathing machine he will notify Ms. Bre or Ms. Natasha that he is not holding up to their agreement to keep it on while working and only remove it to eat lunch. |
