| What duties were performed today? | He was assigned to the stairwell to sweep. He was found in other areas after he started so there is concern that he may not be completing his tasks everyday | ||||||

| Did person have all necessary items to perform job duties? | He is given the proper equipment to perform his duties, there are still concerns of him misplacing his equipment and him wandering off from his equipment | ||||||
| Did the person interact positively with coworkers? | Yes he interacts well but supervisors have noticed that he seems more and more confused when he is approached and directed back to the area he is suppose to be in. | ||||||
| While working, did person take initiative to move on to another task without prompting? | Because he is usually by himself when he does the stairwells it is not certain that he is completing the tasks before wandering off, and when he wanders off he is not taking his equipment with him so its not certain that he is moving on to other tasks | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | Incidents of him wandering off and having to be located and redirected back to his assign area is of utmost concern now along with the confusion he is exhibiting when told of where he needs to be and the task he is to complete | ||||||
| Comments, Office/DDS Follow Up | Ronald his supervisor called Monday and left a message stating that day along he needed to be located three different times and directed back to his assign area. Natasha has spoke to his coordinator about getting him more on shift coaching and it was approved. I contacted Ronald once I heard the news and was only able to leave a message and is waiting for a return call to coordinate with him on the necessary steps to take for a job coach to be onsite with Mr. Brooks | ||||||
| Risk Assessment Monitoring | At risk for wandering while on the job site. He has high blood sugar and he tends to wander off to find snacks and forgets what he is suppose to be doing until one of the supervisors has to come locate him and guide him back to his designated area. | ||||||
