| Did the person arrive on time, if not why? | He arrived at 8:14 am and clocked in at 8:16 am. His driver said that he was going to be ten minutes late picking up, stating that another driver may pick him up but that he was only going to ten minutes up the street if the other driver doesn't shows up and just to give him some time to get here. He ended up arriving ten minutes early and called to let me know he arrived | ||||||
| Was the person appropriately dressed, if not why? | Yes he was dressed appropriately in a black long sleeve button up collared shirt with blue faded Dickie pants and a brown belt. He also had on his black and white tennis shoes with his grey and white raiders winter hat and a blue winter coat. | ||||||
| What duties were performed today? | He cleaned the break room today as well as the freezer area behind the meat department. He swept all numbered aisles and the butchers block and oyster bar area. He swept the rugs in front of the exit doors and his favorite spot the floral area | ||||||

| Did person have all necessary items to perform job duties? | Yes he had all the necessary equipment such as his assigned broom and we found a two wheel dustpan hanging in the back to perform his duties for the day | ||||||
| Did the person interact positively with coworkers? | Yes he positively interacted with his co-workers by greeting them or giving them fist bumps asking them how they were doing | ||||||
| While working, did person take initiative to move on to another task without prompting? | He lets me know by gesturing to what areas he wants to clean and I assist him with making sure he cleans these areas properly. I have started just carrying the dustpan myself to ensure he cleans the debris he can not see | ||||||

| Comments, Office/DDS Follow Up | He had a roast beef and mayo sandwich today that he exclaimed looked real good and he did eat all of it after I took the crust off for him. He also had white cheddar Doritos and a vanilla boost drink and a bottle of water | ||||||
| Risk Assessment Monitoring | At risk for falls due to his gait and collisions with objects and or people due to him not always being aware of his surroundings when he is focused on the tasks he is doing. | ||||||
