| What duties were performed today? | Mr. McAdams duties included maintenance all the men restrooms located in both west wing as well as the east wing of the building, including all 6 floors.His begins his duty by to wipe down toilet and urinals. Restock toiletries, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock wipe the floor if wet as he is leaving out each restroom. He is responsible to maintenance the men restrooms in all six floors including the basement level and both east and west wing of the building as well as cleaning the gym and nursery restrooms located in the basement. | ||

He stocks his cart full of color coded rags to wipe different surfaces, liquid soap to replenish the soap dispenser. Trash bags for the trash, paper towels and toilet paper. Mr. McAdams also had his bookbag containing his oxygen machine. | ||

| Comments, Office/DDS Follow Up | Mr. McAdam complain again about his back and leg paining him while he was doin his shift today. He informed staff while he was walking his right leg felt as if it popped out of the socket or he says he pulled a muscle. He also stated that beuse of the tank he now works with his tank off his back to maintenance the restrooms. However he agreed to put back his oxygen tank once he finished and heads to the next restroom or floor. Mr. Wayne McAdams was ready to call his driver to leave early but ended up working threw it to complete his shit and leaving his regular scheduled time but did inform staff that he would be going to urgent care after work to get his leg looked at and may possibly miss work if his pain persist. | ||
