Refused to work and why? Was another job done in place of this?He did not refuse to work. His supervisor Ronald said that he is arriving and gets his equipment to do his job but once left alone he isn't sure if he is completing his tasks
Did the person arrive on time, if not why?He arrives on time and Ronald also addressed the concern that he has been clocking in early and sometimes clocking out early. I was made aware that last Friday he had clocked out twenty minutes earlier than normal. Also that they have been having difficulty finding him at the end of his shift which he ends up clocking out later than he is supposed to according to both supervisors Freddie and Ronald
Was the person appropriately dressed, if not why?He has been dressing appropriately. During the times that I have seen him he is wearing a blue chimes issued shirt with black jeans, black shoes and a winter hat. Ronald addressed the concern of his hygiene that it would be good for a few days but then goes back to a body odor coming off of him. He has not appeared unkept when I have seen him
What duties were performed today?He was assigned to the stairwell to sweep but concerns have been brought up of him wandering off and him being located in other areas that he isn't assigned to

Did the person interact positively with coworkers?He interacts well with others, he doesn't hold long conversations and gives short answers but typically stays to himself due to his tasks being in the stairwells in the evening that he doesn't come in contact with many people
While working, did person take initiative to move on to another task without prompting?Ronald has addressed the concern that he may not be doing his job because of him wandering off as often as he does as being found on other areas of the building or him misplacing his equipment
Were there any incidents/health related issues/medication side effects or concernsNo incidents have been reported at this time but concerns addressing him wandering off and him actually completing his tasks have been brought up
Comments, Office/DDS Follow UpI spoke to Ronald and he addressed his concerns which I have relayed back to Natasha who is currently in the process of coordinating with his coordinator to adjust the hours he may need for onsite job coaching for all of his shifts and not just the Wednesday shifts
