| What duties were performed today? | The store again was fairly neat so he cleaned all numbered aisles and assisted the floral area clean up as they were restocking their inventory which gave him a particular section to keep up. He also swept the loading and receiving areas | ||||||

| Comments, Office/DDS Follow Up | He had a turkey and mayo sandwich for lunch that he only took two bites of even though I took the crust off for him, he had a bag of baked Cheetos that he only ate a few of, as well as a strawberry yogurt but it was frozen so he did not eat that as well. He had a vanilla boost shake and a bottle of water. I had asked him if he was ok because he normally eats his food and if he doesn't eat anything the least he would do is eat his chips in which he did not. He seemed sluggish this morning when coming in but was active during performing his duties. I also notice that he had a cough throughout the entire shift and not just while he was eating when it normally occurs. He kept saying he was ok and giving me the thumbs up or ok sign when I asked but his sluggishness and him not eating brought concern to me | ||||||
