| Comments, Office/DDS Follow Up | Forest ham and mayo sandwich that they toasted but did not cut the crust off of. Great value tortilla chips, strawberry applesauce, breakfast protein bar and a bottled water he had for lunch. | ||||||
| Risk Assessment Monitoring | Is at fall risk. I made sure he watched where he was going, coaching him to look up when people are around and he is sweeping not being completely aware of his surroundings. | ||||||
