| Refused to work and why? Was another job done in place of this? | No he did not refuse to work, he was distracted by doing other things such as going to the gas station and then to the store downstairs before encouraging him to focus on bettering his communication for job interviews | ||||||

| What duties were performed today? | He explored previous videos he watched last week until I suggested Corporate Chastie where he watched videos on why you're not getting hired and other helpful videos. | ||||||

| Did person have all necessary items to perform job duties? | Yes he brought his bookbag that he carries his laptop and headphones in a long with other items. He did not use his headphones today because I wanted to hear what he was searching so I could keep him on track and or suggest new or other videos. | ||||||

| Comments, Office/DDS Follow Up | He had a doctors appointment and when he did arrive he left right back out to go to the gas station and then to the store downstairs in the office building. Once he did get back he took his time eating while I tried to coach him to watch videos as he was eating just to catch up but he waited till he was finish before getting started with his job exploration. We did go through his emails and voicemails checking to see if any positions responded back with no avail | ||||||
| Risk Assessment Monitoring | Risk of falling, which I worry about when he goes out of the office and the building but he seems to be responsible in moving around on his own in these instances but did address my concerns with him. | ||||||
