| Did the person arrive on time, if not why? | I called the driver at 8:10 asking how far out were him and Sammie, he said they were under the third street tunnel and said once he through that traffic he should be five minutes away. Pulled up at 8:23 am and clocked in at 8:24 am |
| Was the person appropriately dressed, if not why? | Yes he has on the same black button up long sleeve shirt, blue faded Dickie pants with brown belt, he had on his black and white shoes today and he was able to walk better while working, a black grayish winter hat and a heavy blue winter coat |
| What duties were performed today? | Swept break room, butchers block and oyster bar areas. He swept all the numbered aisles. The fresh local produce and farmies harvest area, the floral and cheese shop area |

| Comments, Office/DDS Follow Up | He had forest ham and mayo sandwich that I removed the crust off so he was able to eat entirely. Ruffles potato chips, strawberry applesauce and a coworker gave him a two pack of duplex creme sandwich cookies. He had regular applesauce and whole grain peanut butter crackers which he did not eat. I've asked the driver about him coming on time to pick up Sammie that his late pickups have been interfering with me moving on to my next client of the day. The driver informed me that he has a lady that he is scheduled to pick up at 10:30 am from dialysis, he stated that they don't let her come out until 11 or 11:15 am when he is scheduled to pick up Sammie. He then said that if I have to leave it at 11 am that he suggest I leave Sammie in the Starbucks dining area and that he will come in and get him from there. I explained I am not comfortable with that because anything could happen in between that time. Although he states that every time he is late it is because of this patient but when he comes to get Sammie, there is no other passengers at all except for today and it was a male passenger. I don't understand why the driver would take on these trips when he is aware of the overlapping conflict of time. |
