Did the person arrive on time, if not why?Yes, Mr.McAdams did arrive on time this morning for work. He informed his job coach he was dropped off by his driver 40 minutes early, where he then waited in the McDonald’s a cross from his job site until it was closer to his beginning of his shift .
Was the person appropriately dressed, if not why?Mr. McAdams was appropriately dressed for work wearing his Didlake polo shirt his name tag badge as well as some comfortable sneakers.
What duties were performed today?Mr.McAdams duties included maintenance the mens restrooms. He supposed to wipe down toilet and urinals. Restock toiletries, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock wipe the floor if wet as he is leaving out each restroom. He is responsible to maintenance the men restrooms in all six floors including the basement level and both east and west wing of the building as well as cleaning the gym and nursery public restrooms downstairs.

Did person have all necessary items to perform job duties?Mr. McAdams did have all necessary items to perform his job duties. He stocked his cart full of color coded rags to wipe different surfaces, liquid soap to replenish the soap dispenser. Trash bags for the trash, paper towels and toilet paper as well as a dry mop. Mr. McAdams also had his bookbag containing his breathing machine.
Did the person interact positively with coworkers?Mr. McAdams did interact positively by greeting his colleagues .
While working, did person take initiative to move on to another task without prompting?Mr. McAdams did take initiative moving from one task to the next without prompting from staff or his supervisor.
Were there any incidents/health related issues/medication side effects or concernsNo there weren't any incidents/health related issues/medication side effects or concerns to report on.
Comments, Office/DDS Follow UpMr. McAdams was distracted today after finding out that he was undergoing background check. He was worried that his job maybe in jeopardy because of the background check however staff reminded him to focus on work ahead and to continue doing a good job and everything would be fine. Eventually he decided to listen and not to worry after his supervisor was trying to get Mr. Mcadams to clean the inside of the elevators. However because Mr. Mcadams was not focused he couldn’t demonstrate it properly, his supervisor understood that Mr. Mcadams was distracted and reminded him to lock in and pay attention to do it properly wipe and dust dust the elevator.
