| Was the person appropriately dressed, if not why? | He had on the same black long sleeve button up collar shirt, blue faded Dickies, brown belt, black and white shoes, black and grey zip up hoodie and winter hat. | ||||||
| What duties were performed today? | He started in the front area sweeping next to the restrooms and cashiers. He then went and swept the wellness area along with the emergency exit doors around the building. He swept over near the floral area as well as the self check out area on the other side of the building. | ||||||

| Did person have all necessary items to perform job duties? | Yes he had all the necessary equipment to complete his tasks for the day. I also asked about them ordering his own broom and dustpan for him and I haven't heard anything back yet. | ||||||
| Did the person interact positively with coworkers? | Yes he interacted very well with coworkers, asking them how was their thanksgiving and explaining how he watched the parade on | ||||||
| While working, did person take initiative to move on to another task without prompting? | He needed coaching in some areas, I started using the dustpan and just placing it in front of areas that need to be swept and he knows now that it needs to be swept even though he doesn't see what needs to be swept just to ensure that he is doing his job to the best of his ability and ensuring a clean work environment | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | No incidents to report at this time. | ||||||
| Comments, Office/DDS Follow Up | He had a turkey sandwich, lays baked potato chips, Mott's strawberry applesauce, goldfish crackers, whole grain peanut butter crackers, and bottled water for lunch. I removed the crust and he was able to eat his entire sandwich. He also is having less coughing after eating as I encourage him to chew very well. He still says he feels he does have a hard time swallowing but not specifying what is hard about it. | ||||||
