| Did the person arrive on time, if not why? | called the driver at 8:07 am and he explained he was running late by a few minutes, he pulled up at 8:24 am and Sammie clocked in at 8:25 am. The driver explained that the house was having issues with getting Sammie out the house. | ||||||
| Was the person appropriately dressed, if not why? | He wore the same black button up collar shirt, blue faded dickie pants, brown belt, black and white shoes, and a blue zip up jacket with a winter hat. | ||||||
| What duties were performed today? | He started in the break room and swept the back loading area behind the meat department, he swept all the rugs leading from all back areas around the building, he swept down all the aisles, the floral area and the check out area on the other side of the building. | ||||||

| Did the person interact positively with coworkers? | He interacted very well with his employees. Still have my concerns on how certain ones interact with him when he speaks, there are times they will tell him to back up even though he isn't in their personal space just waving to get their attention and to say good morning, or hello or how you doing. He does give fist bump to some employees and then there are others who would tell him not to touch them even though I have witnessed them first bumping him before. | ||||||
| While working, did person take initiative to move on to another task without prompting? | He knows what areas he wants to clean I just assist him in spots he doesn't feel need to be swept because he can't see the crumb and smaller debris. | ||||||

| Comments, Office/DDS Follow Up | He had a turkey and forest ham and mayo sandwich, cheetos baked, applesauce, vanilla boost drink and bottled water. He didn’t eat his sandwich at all. Ate the cheetos and applesauce. He coughed some but I just reminded him to chew his food completely. I asked him does he like the crust on his sandwiches he said no so I removed it and it was the first time I seen him eat the whole sandwich and he was more than happy to be able to and keep giving me the ok hand gesture or the thumbs up when I asked was that better. | ||||||
