| What duties were performed today? | was able to complete his tasks by sweeping the back loading areas, meat department, frozen aisle, vegetable areas, floral areas, cheese all around the world, and dairy aisle. | ||||||

| Did person have all necessary items to perform job duties? | No they didn’t have the normal equipment that he needed to work. He refuses to work with certain brooms, either they are too big or the brush itself isn’t neat enough for him, and he likes to use the dustpans with two wheels on it, most in the building have broken wheels or one wheel dustpans. The individual he was in use of the equipment at the time refused to allow him to use it then said she doesn't speak english. Another of his coworkers suggested I go back to the young lady in charge of orders to explain the situation and she went herself and retrieved the broom and dustpan from her. Once he received the necessary equipment he then was able to complete his tasks for the day. | ||||||
| Did the person interact positively with coworkers? | He interacted with his coworkers very well. He was very excited about thanksgiving coming up and was wishing everyone a happy thanksgiving. | ||||||
| While working, did person take initiative to move on to another task without prompting? | Yes he knows what areas he wants to clean but once getting to those areas if it isn't large debris he believes it doesn't need to be done and I will have to Coach him to those particular areas by using my foot to move the what he can't see so he can sweep it. | ||||||

| Comments, Office/DDS Follow Up | He had a Turkey and Mayo sandwich, barbecue baked lays chips, and two bottles of water. He was coughing after eating today. I had asked him if it was hard to swallow and explained that it is. At times as he was eating he would stretch his neck up towards the ceiling which made me ask that. I explained that he should make sure he chews his food completely before swallowing and he stated he would. Encouraged him to drink when he felt it was hard to swallow to assist the food going down. There was less coughing after this. Also notice he takes only a few bites of his sandwich before he says he is done with it and would eat everything else but the sandwich. The driver was late picking him up. I called him at 11:14 am and he said he was on his way and pulled up at 12 pm. | ||||||
| Risk Assessment Monitoring | No incidents to report. Will continue to monitor him for fall prevention and watch him while he eats to ensure he doesn’t choke on any food. | ||||||
