| Was the person appropriately dressed, if not why? | He had on the same black long sleeve button up collar shirt with faded blue Dickies, brown belt, black and white shoes, black and grey zip up hoodie. | ||||||
| What duties were performed today? | He was able to find the broom and a two wheeled dustpan for him to use. He swept the bistro area, the floral area, cheese all around the world area, the wellness area, break room and back loading dock. | ||||||

| Did person have all necessary items to perform job duties? | He was able to find the broom and a two wheeled dustpan for him to use. Everyone and the manager is aware he only likes to use particular equipment and there are certain employees who would take it and when asked if we could trade them with a different broom they have been difficult. I did speak with the young lady who orders equipment and she said she was going to look into ordering him a broom which they label and it is just used by him. | ||||||
| Did the person interact positively with coworkers? | He interacts very positively with his employees. I do have concerns on how some may treat him back, especially when it comes to the equipment that he uses | ||||||
| While working, did person take initiative to move on to another task without prompting? | He did need coaching on sweeping areas where he looks and feels there's nothing to be swept because there isn’t large debris. I have been working with different techniques to guide him to areas he usually says no to. I have suggested holding the dustpan near certain areas and he caught on to that. I was doing that because it needed to be swept and he would sweep it and notice that the area did need to be swept. | ||||||

| Comments, Office/DDS Follow Up | Had a Turkey and mayo sandwich, goldfish, honeybun, strawberry applesauce, strawberry yogurt that he did not eat because he didn’t have a spoon, and a bottle of water. I have noticed after eating these coughs he gets but he explains he is ok but it happens very often after eating his lunch. | ||||||
