| Refused to work and why? Was another job done in place of this? | He did not refuse to work, he came in a little tired he said but once he had his equipment in hand he became focused on his job. | ||||||
| Did the person arrive on time, if not why? | No he did not arrive on time. Called the driver at 8:13 am and said he was pulling up. He clocked in at 8:16 am | ||||||
| Was the person appropriately dressed, if not why? | Yes, although he had on the same long sleeve button up collar shirt, faded blue denim pants, black and white tennis shoes, brown belt and black and grey zip up hoodie, he was dressed appropriately for work. | ||||||
| What duties were performed today? | He started in the break room working his way around the back storage area before going onto the floor where he swept the bistro area and floral department before moving on to the Cheese all around the world area. He swept the entire wellness area before taking his lunch at 10:30 am. | ||||||

| Did person have all necessary items to perform job duties? | Yes he had all the necessary equipment to complete his tasks for the day. | ||||||
| Did the person interact positively with coworkers? | Yes, he speaks to employees as he passes by at times some speaking back to him and others ignoring him completely. He has great interactions with the security team as well. | ||||||
| While working, did person take initiative to move on to another task without prompting? | He still needs coaching on sweeping particular areas to ensure he is doing the task properly. He has a habit of saying he don't see anything, or he did certain areas that either he hasn't done yet or do need to be swept and him refusing until I gesture to the mess in question. | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | NO incidents occurred on this shift. I have concerns about this cough he gets after he is finish eating and sitting at the table. It will go away once he begins walking but not sure if anything he is eating is causing him to have these coughing sessions. | ||||||
| Comments, Office/DDS Follow Up | The driver called at 8:34 am after dropping him off suggesting that he would be a few minutes late on picking him up but wasn't sure because he said he may be on time but he has other pickups. He didn't give an estimated time on his arrival. Sammy clocked out at 11:01 am and the driver did not arrive until 12 pm. I called the driver at 11:34 am and he said he was a few minutes away. I called again at 12 noon when he still hadn't shown up yet and it was at this time that he pulled up. He also has not had me signing the pickup and drop off sheet since the conversation about not being comfortable with signing for days in advance. Sammy ate a turkey and mayo sandwich, goldfish and a honey bun. He has strawberry yogurt and a strawberry applesauce but no spoon was packed with his lunch so he set it aside for when he got home. Sammy also at the end of his shift went and purchased himself a coca cola with the five dollars he had in his pocket. | ||||||
