| Refused to work and why? Was another job done in place of this? | He did not refuse to work today. Driver said he had a rough morning but as soon as we got in before i was able to clock him in he was looking for the broom and dustpan to get to work. |
| Did the person arrive on time, if not why? | No he did not. I called the driver at 8:30 am to see if they were on their way and he said he was going to explain when he pulled up and hung up. When the driver did arrive he said it was a mess at the house and they had to wait thirty minutes for Sammy to get out of the house, He did have two other passengers with him in which he stated because of Sammy they he was late for their drop offs. Didn't arrive until 8:40 am. |
| Was the person appropriately dressed, if not why? | Yes he had on a black button up collar shirt with blue faded dickie pants and a brown belt. He had on black and white shoes as well as a black and grey sip up hoodie |
| What duties were performed today? | He swept down all the aisles and floral department. He swept in the old security room and the back exit door areas. He was very focused and eager to get to work and stay working knowing that he was late. He only used the bathroom twice during this shift. |

| Did person have all necessary items to perform job duties? | It's been difficult finding the certain broom and dustpans he likes to use, he does refuse to use the brooms with the thick bristles or if they are messed up. His coworkers are aware of this and we were able to find it with another coworker but they seemed upset even though they gave the broom to Sammy anyway. |
| Did the person interact positively with coworkers? | He interacted well with his coworkers today I noticed that a few of his coworkers weren't being positive to him. Especially with the broom incident, I believe if i wasn't there to advocate for him that the employee would have been negative towards him. |
| While working, did person take initiative to move on to another task without prompting? | Yes he guided me to areas he wanted to get to because he wanted to make sure he got as much as he can done in the little time that he had because of the fact he was late, stating he will get it done and that he wasn't old yet. |
| Were there any incidents/health related issues/medication side effects or concerns | The only incident I will like to bring to attention is the way his coworkers have been acting with the particular equipment he like to use to complete his tasks, and finding these particular ones wit them as they have said and are aware that he likes to use this particular sets and they giving him grievance |
| Comments, Office/DDS Follow Up | He had a toasted ham sandwich with mayonnaise, harvest cheddar sun chips, cupcakes, activia yogurt (he did not eat), peaches fruit cup (he did not eat, a banana (he did not eat) and a bottled of water. At the end of his shift I walked with him so he could purchase him a coca cola, I coached him on how to scan it and insert his money as well as take his receipt and put it and his change in his pocket. First day in weeks he was excited to buy himself something after work. |
