| Refused to work and why? Was another job done in place of this? | No he didn't refuse to work. There are certain areas of the store especially under certain shelves he won't sweep under because he physically can't see the debris, I have to convince him to check otherwise he won't do the area. | ||||||
| Did the person arrive on time, if not why? | No his driver didn't arrive till 8:28 am, he had no other passengers with them either. Driver just said it was a mess trying to get him out the home that day. He clocked in at 8:30 am. | ||||||
| Was the person appropriately dressed, if not why? | Yes he had on a black button up collar shirt with faded blue dickie pants on with a brown belt. He wore black and white shoes along with a black and grey zip up hoodie. | ||||||
| What duties were performed today? | He swept the floral department and vegetable departments. He swept in the back loading and unloading area. Had to use the restroom three times during this period. Swept the aisles near the cashiers and customer service desk. | ||||||

| Did person have all necessary items to perform job duties? | We had to walk the store twice before we were able to find the broom and dustpan that he is comfortable with using, but eventually he had all the equipment necessary to complete his tasks | ||||||
| Did the person interact positively with coworkers? | Yes he is cheerful when he greets coworkers to ask how they are doing and to let them know that he is doing pretty good. | ||||||
| While working, did person take initiative to move on to another task without prompting? | He needed prompting in certain areas of the store but still was able to guide me to areas he felt needed to be taken vare of. | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | No incidents occurred today | ||||||
| Comments, Office/DDS Follow Up | Had a slight cough today along with a runny nose, he wore a mask through this shift. When asked if he was ok durinn these coughs he would say ok are put his had up making the ok gesture with his hands. He had a ham and mayo sandwich, cheetos, peach fruit cup, vanilla boost drink and a bottled water. His driver was late picking up that day as well. He has not asked me to sign any paperwork for picking up and dropping Sammy off. | ||||||
| Risk Assessment Monitoring | Monitor for falls as he is cleaning around the store because he can become very focus and not pay attention to his surroundings. making sure he doesn't bump into anyone or hit anyone with the broom. | ||||||
