| Refused to work and why? Was another job done in place of this? | Mr. White did not refuse to work, he came in eager to get started. | ||||||
| Did the person arrive on time, if not why? | No he did not arrive on time. The driver didn't drop him off at 8:19 am, The driver came late to pick him up as well. The driver called my phone at 10:49 am that I missed. I called him back at 10:58 am when I noticed I missed the call and he did not answer. He Then called me right back stating that he thought he was going to be late but then stated he was only a few minutes away. Sammy and I stood at the door waiting for his driver, it was cold outside and Sammy did not want to wait outside in the cold so we watched from the door for his arrival. The driver called me at 11:23 am and before I could answer he hung up. He pulled up at 11:25 am and stated that he had been circling around but he wasn't. Also he hasn't been allowing me to sign Mr. Whites pick up and drop off sheet, either he doesn't ask or he doesn't have the clip board with him. Sammy clocked in at 8:20 am. | ||||||
| Was the person appropriately dressed, if not why? | Yes, he had on a black button up coolar shirt, black dickie pants, black and white shoes, and a grey and black zip up hoodie. | ||||||
| What duties were performed today? | He started in the front after we searched the store for a dustpan. He then swept in front of the pharmacy and dairy areas. He swept the rugs in front of the doors where crates come in and out. He swept down the cleaning aisle and the can good aisle. He also swept near the floral department. He spent majority of his time sweeping infront of the main entrance because of the wind blowing in all the leaves. He swept the dining area and near the self serve food as well. He needed coaching to sweep under certain tables that needed cleaning but because he wasn't able to see what needed to be swept he would say he already did it. i started taking my foot and sweeping out the crumbs or debris from these areas so he can see that it does need to be swept. | ||||||

| Did person have all necessary items to perform job duties? | We were able to find a broom in the break room when he first arrived and sat down hi bag, but wasn't able to find a dustpan till we walked to the front of the store. During one of his bathroom breaks, one of his coworkers that saw him using this set came and took it from him. We had to then go back around the store looking for another set. He doesn't care to use the dustpans that don't have the wheels on it, we were able to find one next to the elevators near the floral department. | ||||||
| Did the person interact positively with coworkers? | He was speaking to all the employees, the employees were dressed up for halloween so he was telling co-workes and patrons trick or treat and happy halloween. He was very talkative and interactive today. | ||||||
| While working, did person take initiative to move on to another task without prompting? | In most cases he was able to voice the areas he wanted to go to. He did need coaching to sweep certain areas. Sometimes i would hold the dustpan while he sweep certain areas and for areas that need to be swept that he didnt see, if I would place the dustpan facing the debris he started to catch on that those areas needed to be cleaned | ||||||

| Comments, Office/DDS Follow Up | Sammy wanted a coca cola at the end of the day, I asked him if he had any money and he said no and driver. I am aware that he is suppose to have his own money on him for food or other items while he is at work, and he does always want a coca cola at the end of the day but he has not been having any funds on him for the past few weeks. | ||||||
