| Refused to work and why? Was another job done in place of this? | No he did not refuse to work. Driver was late dropping him off and he stated he had a hard time getting Sammy out the house, he said he called the house multiple times and ended up leaving there late. | ||||||
| Did the person arrive on time, if not why? | No, I had to call the driver at 8:21 am to see if they were on their way, he answered and said he was at the light. When he did pull up he explained he had a hard time getting Sammy out of the house, that he called the house numerous times and ended up leaving late. Clocked it at 8:24 am | ||||||
| Was the person appropriately dressed, if not why? | Yes he had on a pullover button collar shirt with black dickie pants with belt, black and white shoes and a gray and black zip up hoodie | ||||||
| What duties were performed today? | He started his tasks in the break room where he swept entirely, then the back areas where the freezers are at. He then went around the whole store sweeping areas as needed. His favorite area to sweep is near the flower desk, they always have leaves, petals and dirt to clean up and I believe he is aware that area he has to clean frequently, that when I do ask which way he would like to go that area he points to often. | ||||||

| Did person have all necessary items to perform job duties? | Yes there was a broom in the breakout where he began sweeping but had to go through the back area searching for the dustpan so he can complete his duties | ||||||
| Did the person interact positively with coworkers? | Yes, he was very talkative today and interacted well with his coworkers. | ||||||
| While working, did person take initiative to move on to another task without prompting? | He needed some promoting to move to other areas he had been distracted but food on the shelves and the open bar area with the fresh cooked food. He said he wish he could get some of that one day. | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | No incidents occurred while on duty. I have observed a cough when he eats buy he says he is ok but he tends to cough often when he eats. | ||||||
| Comments, Office/DDS Follow Up | He had a ham and mayo sandwich which he only took two bites of, baked cheetos which he ate all of, a vanilla boost drink he drank all, a peach yogurt which he ate all and a small bottle of water which he drank all of it. Also driver did not have me sign the clipboard for drop off or pick up | ||||||
| Risk Assessment Monitoring | Monitored his steps throughout the store to keep from slipping or falling. Had to coach him to watch out for people as he was walking through, when he is focus on looking for stuff to clean he sometimes doesn't look up to watch his surroundings | ||||||
