| Refused to work and why? Was another job done in place of this? | Mr. White did not refuse to work. He came in a little sluggish but once he hot the broom and dustpan in his hands he was off to work | ||||||

| Was the person appropriately dressed, if not why? | Yes. Black button up collared shirt, faded blue dickie pants, black and white slip in shoes and a gray and black zip up hoodie. I suggested to the driver to ask the house to put a hat on him since its getting colder. | ||||||
| What duties were performed today? | He swept the entire store starting in the back freezer area. He especially likes Sweeping bear the flower area of the store. Other Areas he'll say he has done it already but that's the only area of the store he didn't mind sweeping over again. | ||||||

| Did the person interact positively with coworkers? | Yes He greeted them and asked them how they were doing. He was telling me how he knows his coworker and a how hard they work | ||||||
| While working, did person take initiative to move on to another task without prompting? | I had to coach him to sweep areas he didn't sweep because he would say he already did that area but it needed to be swept, I would have to use my shoe to move the debris so that it would encourage him to clean the area even though he stated he swept it. | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | No incidents Occurred today. All went smoothly. | ||||||
| Comments, Office/DDS Follow Up | He had a turkey and mayonnaise sandwich, smart white cheddar popcorn, applesauce, water and an orange Gatorade for lunch. In the Seafood area he can be distracted by the fish, shrimp and crab legs behind the display, have to give him a friendly reminder of the work he is supposed to be doing. | ||||||
| Risk Assessment Monitoring | Monitor his steps ensuring no falls or slips as well as making sure he didn't hit anyone with the broom while sweeping. Once he is in motion it can be difficult to pay attention to his surroundings | ||||||
