Was the person appropriately dressed, if not why?Yes, he had on a black button up collar shirt with a light zip up jacket, with black dickie jeans and black shoes. It appears it the same outfit every time but it is clean.
What duties were performed today?Mr. Sammy swept the main part of the store. They were doing a lot of shipment this morning in the back area so he did not sweep back there at all. He went down every aisle

Did person have all necessary items to perform job duties?We took a few minutes looking for the broom and dustpan. A coworker gave him a set with a dustpan that had no wheels, and the broom. As we started through the store we eventually found a dustpan with wheels as well as the broom he likes using.
Did the person interact positively with coworkers?Yes, spoke to coworkers as they passed. One coworker shared some over her pretzel snacks with you and he enjoyed them had a short conversation and went straight back to work.
While working, did person take initiative to move on to another task without prompting?He would direct me to what areas he felt he hasn't gotten to yet without promoting. Certain aisles he could not see some of the debris so again had to coach him to sweep areas that he didn't see trash at.
Were there any incidents/health related issues/medication side effects or concernsA group of young boys about seven of them, started running up to him yelling Sir and antagonizing him. I stepped in front of him and told them don't do that, they began to speak, when i just put my hand up and calmly told them not to do that. Some apologize and the others started antagonizing other patrons.
Comments, Office/DDS Follow UpHe did not notice the young boys bothering him I deescalated the issue before he could understand what is going on. He verbally was speaking about Thanksgiving and being able to watch the parade on tv.
Risk Assessment MonitoringMonitor him to make sure he doesn't fall or slip or any areas that had water on the floors.
