| Did the person arrive on time, if not why? | No the driver dropped him off at 8:13 and he didn't clock in until 8:14. Driver did not give a reason as to why he arrived late. | ||||||
| Was the person appropriately dressed, if not why? | Yes, he had on black dickie pants and a black button up collared shirt. He had on black and white shoes as well as a black and white zip up sweat jacket. | ||||||
| What duties were performed today? | We sat his bag down and found a broom in the break room but had to go look for the dustpan which was found near the freezer area, where he then started sweeping and collecting debris. He then went out into the main area of the store starting in the dairy and freezer department. He swept the front area where the flowers are. Cleaned the sitting area where patrons eat, he removed the chairs to sweep under and around the tables as well. | ||||||

| Did the person interact positively with coworkers? | Yes, he greeted many of his coworkers and had some laughs and back and forth banter with them. They told him he was doing a good job and he would repeat that he was doing a good job and saying he has to be strong to do the job. | ||||||
| While working, did person take initiative to move on to another task without prompting? | He focused on what he could see but i had to coach him to some areas where he couldn't physically see the debris because it was small but coached him into using the broom to sweep over areas to see if anything moves which helped him to clean certain areas more thoroughly. | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | When he was eating his lunch he took a bite of the sandwich and began spitting it back out, he drank some water and began spiting up in the trash can. He said he wasn't choking cause he was able to speak but said it didn't taste right. He started having a coughing fit as if he was trying to dislodge something from his throat. i kept asking if he was ok and he kept insisting that he was ok. I had him wash his face and hands and he went back to eating but didn't eat anymore of his ham sandwich with mayonnaise. he had a ham sandwich with mayonnaise that he didn't finish, barbecue potato chips, ahoy cookies and a pineapple fruit cup with water and a vanilla boost drink. | ||||||
| Comments, Office/DDS Follow Up | He becomes anxious when he sees an area that needs to be swept but i find to be too much traffic for him to sweep at that time and convincing him to come back takes time. He was in the floral section of the store and behind the counter was a big mess, but the young lady was working with a customer and was going back and forth as i was trying to convince him to come back once she finished he did get anxious and she told him to go ahead and sweep, but then kept complaining to him not to sweep her feet, but once he starts he becomes focus on the mess and not the people around him. i was trying to avoid her complaint by coaching him to come back but she gave him the go ahead and still complained to him. | ||||||
| Risk Assessment Monitoring | I monitored his surroundings ensuring to remove him from high traffic areas as to risk him hitting anyone with the broom as well as making sure he doesn't slip or fall while he is doing his task. | ||||||
