| Did the person arrive on time, if not why? | The driver said he had to wait for Mr. Whites lunch to be done. Driver explained that he can be late from time to time due to the house not having him completely ready. | ||||||
| Was the person appropriately dressed, if not why? | Sammie was appropriately dressed and groomed for work. | ||||||
| What duties were performed today? | Sammie started off in the back area sweeping. He also swept out on the main floor and went down every aisle sweeping. | ||||||

| Did person have all necessary items to perform job duties? | Yes there were multiple brooms and dust pans for him to use throughout the store. He did switch out his dustpan when he felt one was not properly rolling out for him. | ||||||
| Did the person interact positively with coworkers? | Yes, he greeted and fist dapped many employees. Many employees came to him and spoke with him as well. He spoke but maintained focus on doing his job duties as well. Some he was so focused on the task that he didn't notice they spoke and they were very understanding of that. | ||||||
| While working, did person take initiative to move on to another task without prompting? | Yes, I simply would ask him which way he wanted to go, he would direct me with a head gesture of the direction he wanted to go in and he would follow behind focusing on the floor. There was debris that blended in with the flooring, where I would stand next to the debris to see if he would notice it without telling him it's there and that helped him sweep more effectively. There are times when certain debris was plastic and he couldn't tell if it was the floor or not. I would step on the piece and have it move so that he would understand that it was trash for him to pick up. There are times when he would ask me if it is trash and then I would let him know if it was or not. I would walk pass areas and ask him if it looks good, and if he felt it didn't or was unsure he would sweep to see if anything would move and would comment back to me "Looking good". This also helped with ensuring he was doing his job to the best of his ability. | ||||||
| Were there any incidents/health related issues/medication side effects or concerns | No incidents occurred. He did go to the restroom three times throughout his shift. | ||||||
| Comments, Office/DDS Follow Up | I notice he doesn't like sweeping up pieces of paper that are notebook sized and not crumpled up. I began crumpling them up for him so he could recognize it as trash. | ||||||
| Risk Assessment Monitoring | Making sure he is aware of his surroundings, to watch out for customers walking and moving around him as he swept. | ||||||
