| Did the person arrive on time, if not why? | Mr.McAdams did arrive on time today morning for work. He informed his job coach he was dropped off by his driver 8:45 but he was able to clock in as soon as he arrived for work this morining | ||||||
| Was the person appropriately dressed, if not why? | Mr. McAdams was appropriately dressed for work wearing his Didlake polo shirt his name tag badge as well as some comfortable sneakers and his backpack with his oxygen machine and a spare tank in case one finished before end. | ||||||
| What duties were performed today? | Mr. McAdams duties included maintenance all the men restrooms located in both west wing as well as the east wing of the building, including all 6 floors.His begins his duty by to wipe down toilet and urinals. Restock toiletries, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock wipe the floor if wet as he is leaving out each restroom. He is responsible to maintenance the men restrooms in all six floors including the basement level and both east and west side of the building | ||||||

He stocks his cart full of color coded rags to wipe different surfaces, liquid soap to replenish the soap dispenser. Trash bags for the trash, paper towels and toilet paper. Mr. McAdams also had his bookbag containing his extra oxygen machine. | ||||||

| Comments, Office/DDS Follow Up | Mr. McAdams completed all his duties with cleaning both men and women's restrooms n the west side and east side front basement to the 6 floor. Mr. McAdams got to work on time this afternoon, he wore his gloves and oxygen tank as well but stop wearing his tank near the second half of this shift. Mr. McAdams did let his personal affairs distract him his last hour but by that time he was already managed to complete the majority and had only 4 floors left. Staff did his best to redirect Mr. McAdams into getting back focused and not to let his personal business clout his judgement or effect his proformance. Despite that Mr. McAdams did have a go day even getting praise for a good job. | ||||||
