| Did the person arrive on time, if not why? | Yes, Mr.McAdams did arrive on time this morning for work. He informed his job coach he was dropped off by his driver 8:38 but he was able to clock in 10 minutes early for work this morning. | ||||||
| Was the person appropriately dressed, if not why? | Mr. McAdams was appropriately dressed for work wearing his Didlake polo shirt his name tag badge as well as some comfortable sneakers and his backpack with his oxygen machine. | ||||||
| What duties were performed today? | Mr.McAdams duties included maintenance the mens and women's restrooms. He would begin by announcing himself as janitorial before he enters the women's restrooms as well as the mens. His begins his duty by to wipe down toilet and urinals. Restock toiletries, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock wipe the floor if wet as he is leaving out each restroom. He is responsible to maintenance the men restrooms in all six floors including the basement level and both east and west wing of the building as well as cleaning the gym and nursery public restrooms downstairs. | ||||||

| Did person have all necessary items to perform job duties? | Mr. McAdams did have all necessary items to perform his job duties. He stocked his cart full of color coded rags to wipe different surfaces, liquid soap to replenish the soap dispenser. Trash bags for the trash, paper towels and toilet paper as well as a dry mop. Mr. McAdams also had his bookbag containing his oxygen machine. | ||||||
| Did the person interact positively with coworkers? | Yes, Mr. McAdams did interact positively. Mr. McAdams made effort to speak to his coworker that he had prior disagreement with by starting conversation trying to put in effort to move past this issue. Mr McAdams continues to present the same positivity with coworkers by greeting his coworkers and joking around just being cordial. | ||||||
| While working, did person take initiative to move on to another task without prompting? | Mr. McAdams did take initiative moving from one task to the next without prompting because he is fully aware of his responsibility as janitorial for Didlake. | ||||||

| Comments, Office/DDS Follow Up | Mr. McAdams was happy and relieved to back to work after being sent home early the day before due to his residency having bedbugs because of his roommate old bed set. Mr. McAdams was in good mood but did mentioned his leg muscles was bothering him and causing him to take extra breaks to rest his leg. Mr. McAdams was able to complete all his duties including all mens and women's restrooms form all 6 floors as well as both east and west side. Mr. McAdams was excited about the weekend and told staff his plans since he staying at a hotel until his residency is fumigated against the bed bugs. Mr. McAdams also took it upon himself to try to ment his relationship with one of his coworkers after lashing out then based on feeling disrespected. Mr. McAdam didn't like the tone his coworkers used to tell him to sit down and relax however Mr. McAdams lashed out and yelled at his coworkers to say that the person isn't the boss or parent therefore they can't tell him what to do. That made it a little awkward when they passed or was in the same vicinity. Staff tried calm Mr. Adams down after however with some time passing he was more receptive to the suggestion from staff to ment the relationship with his coworker because the message was coming from a place of love; after a few days today Mr. McAdams did manage to get over it and spark a conversation because this is team at Didlake. | ||||||
