| Did the person arrive on time, if not why? | Mr.McAdams did not arrive on time today morning for work. He informed his job that he was waiting for his note so he could return back to work coach he was dropped off by his driver 10:05 but he was able to clock in as soon as he arrived for work this morining. | ||||||
| Was the person appropriately dressed, if not why? | Mr. McAdams was appropriately dressed for work wearing his Didlake polo shirt his name tag badge as well as some comfortable sneakers and his backpack with his oxygen machine and a spare tank in case one finished. | ||||||
| What duties were performed today? | Mr. McAdams duties included maintenance all the men restrooms located in both west wing as well as the east wing of the building, including all 6 floors.His begins his duty by to wipe down toilet and urinals. Restock toiletries, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock wipe the floor if wet as he is leaving out each restroom. He is responsible to maintenance the men restrooms in all six floors including the basement level and both east and west side of the building as well as the gym and day care restrooms | ||||||

| Did person have all necessary items to perform job duties? | Mr. McAdams did have all necessary items to perform his job duties. He stocks his cart full of color coded rags to wipe different surfaces, liquid soap to replenish the soap dispenser. Trash bags for the trash, paper towels and toilet paper. Mr. McAdams also had his bookbag containing his oxygen machine | ||||||
| Did the person interact positively with coworkers? | Mr. McAdams did interact positively with coworkers by speaking to everyone he comes in contact with with a smile on his face except for one coworker he got into with for suggesting Mr. McAdams sit down. They particular coworkers he don't interact with at all. | ||||||
| While working, did person take initiative to move on to another task without prompting? | Mr. McAdams did take initiative moving from one task to the next without prompting because he is fully aware of what is expected from him working with Didlake janitorial. | ||||||

| Comments, Office/DDS Follow Up | Mr. McAdams was relieved he was able to return back to work after the bedbugs situation at his residence. Mr. McAdams been staying at a hotel until his place is fumigated but he had to provide a letter to his supervisor to insure that he not here just so he could come back to work to prevent the spread of bedbugs to this building. Even though he was late due to waiting for note, Mr. McAdams has been diligently working completing all his duties. Mr.McAdams was able to clean all the restrooms both men & women's on the west side as well as the gym and daycare. He also cleaned all the mens restrooms in the east side but left the women's restrooms for his coworkers who come in the night shift to give them something to do. Mr. McAdams wore his tank and gloves this afternoon | ||||||
