Did the person interact positively with coworkers? | Mr. Gore had an upbeat disposition and was chatty with his co-workers. |
While working, did person take initiative to move on to another task without prompting? | For most of the shift Mr. Gore transitioned smoothly. There were two occasions where I had to remind him to refocus on his tasks. |
Were there any incidents/health related issues/medication side effects or concerns | There were no incidents during the shift. |
Comments, Office/DDS Follow Up | Mr. Gore is adjusting well to his new duties (from vacuuming to mopping). He does need reminders about watching for spills and making sure the utility room floor is not wet. He also needed to be reminded to watch the low ceiling in the basement when using the mop. His good attitude seems to be consistent. |
