Mr. McAdams will learn to use coping skills when he is given new tasks and directives from his supervisor with verbal prompts three times per week through 2/20/2025.

Refused to work and why? Was another job done in place of this?Mr.McAdams did not refuse any work that was assigned to him.
Did the person arrive on time, if not why?Mr.McAdams did not arrive on time for work. Mr.McAdams informed his job coach that he was 10 minutes late due to his driver not picking him up. Mr. McAdams then took it upon himself to take metro transit to get to work this morning.
Was the person appropriately dressed, if not why?Mr. McAdams was appropriately dressed for work with his Didlake uniform, his oxygen tank, and his glasses.
What duties were performed today?Mr.McAdams duties included maintenance both mens and women's restrooms for all six floors from both west and east side of the building. He supposed to wipe down toilet and urinals. Restock toiletries, soap dispenser, wipe the mirrors and sink from water or fingerprints. He also empty the trash and restock the toiletries then he mops the floor if still wet spots.

Did person have all necessary items to perform job duties?Mr. McAdams did have all necessary items to perform his job duties. The items included the various color rags be use to clean with. His glasses on gloves, tissue and paper towels. His glasses and oxygen tank as well to help him perform his duties as efficiently as possible.
Did the person interact positively with coworkers?Mr. McAdams did interact positively with coworkers making jokes as well as small talk depending on who he sees.
While working, did person take initiative to move on to another task without prompting?Mr. McAdams did take initiative moving from one task to the next without verbal promoting from staff or his supervisor because Mr. McAdams is fully competent to understand what is expected from him as a Didlake.
Were there any incidents/health related issues/medication side effects or concernsNo incidents/health related issues/medication side effects or concerns regarding Mr. McAdams.
Comments, Office/DDS Follow UpMr. McAdams returned back to work feeling better. He told staff he was took off because his body was worn out but staff reminded him to not over work himself and remember to pace and take as many breaks as needed. However Mr. McAdams did reveal that there was a situation this morning where Mr. McAdams told staff that his house staff called him the N* word. This effect Mr. McAdams so much so that he asked to get in contact with his behavioral support team & dda coordinator to inform him and possibly help suggest how to move forward since as well as to report the incident. Mr. McAdams leaving early today for a doctor appointment regarding his heart and lungs 4 hour early. Mr. McAdams felt a lil guilty not taking the day off after informing his supervisor he would have to leave even earlier then expected due to his doctor appointment. He was suggested to stay home as well but refused to due his strong work ethic he did inform staff that since he was present Thursday and Friday he would at least come to work and do as much as he can possibly can manage before his departure to his appointment.
